2010 Centennial Quality Commitment and Achievement Form for Unit Award (No. 513-190)
Between October 31, 2008 and January 31, 2009, the district leadership should meet with the leadership of all units to discuss and complete the 2009 Centennial Quality Unit Award Commitment form. This form can be completed by hand, on a typewriter, or you can enter all of the data on the computer by opening this link. You are able to type in the form, save the entered data, and then come back to your saved form, or send it to someone else with the blanks filled in. The "Interpretations" section is on the second page of the link and provides additional interpretation about the achievement for each criteria. You will use this same form and complete the "Achievement" column at the end of 2009 after you have qualified after October 31, 2009, but before December 31, 2009.
